Pharmacology

Dexamethasone is a glucocorticoid that reduces inflammation by preventing white blood cell migration and reversing improved capillary permeability. It suppresses the normal response of the immune system.

Dosage of Roxadex Injection

Dexamethasone can be administered through IV, IM, or local injections. Infusion fluids can also be used to dilute the injections or inject them directly into the infusion line.

For IV injections, large doses should be given slowly over several minutes. Deep intramuscular injection is recommended to prevent subcutaneous adipose tissue atrophy. Intra-articular injections must be given under strict aseptic conditions as glucocorticoids decrease resistance to infection. Sodium chloride 0.9%, Anhydrous glucose 5%, Invert sugar 10%, Sorbitol 5%, Ringer's solution, Hartman's solution (Ringer-lactate), and other infusion fluids can be used to dilute Dexamethasone, which remains potent for at least 24 hours at room temperature and in daylight.

The dosage of Dexamethasone depends on the severity of the condition and the patient's response. For systemic therapy in adults, daily doses of 0.05-0.20 mg/kg body weight are usually sufficient. In emergencies, substantially higher doses are required. An initial dose of 10-20 mg IV is followed by 6 mg IV or IM every 6 hours until a satisfactory result is obtained. The dosage must then be gradually tapered off.

For local therapy, the recommended doses are 2-4 mg for large joints and 0.8-1 mg for small joints in intra-articular injections, 2-4 mg in intranasal injections, and 0.4-1 mg in tendon sheaths injections. The frequency of these injections may vary from every 3-5 days to every 2-3 weeks.

Administration of Roxadex Injection
  • Roxadex Injection will be given to you only by a doctor or a nurse into the vein, muscle, joints, or tendons depending upon the site of inflammation.
  • Your doctor will decide the correct dose and duration of therapy for you depending on your age, body weight, and disease condition.
Interaction

It is important to inform your doctor about any medications that you are taking before receiving Roxadex Injection. This includes:

  • Aminoglutethimide or cyclophosphamide, which is used for treating cancer
  • Anticoagulants such as acenocoumarol, phenindione, and warfarin, which are used for blood thinning
  • Medicines used to treat bacterial infections, such as isoniazid, erythromycin, clarithromycin, and troleandomycin
  • Medicines used to treat diabetes, such as metformin and glipizide
  • Non-steroidal anti-inflammatory medicines, such as aspirin and ibuprofen, which are used for fever and pain relief
  • Medicines used to treat epilepsy, such as carbamazepine, phenytoin, and primidone

  • Diuretics, such as furosemide, chlorothiazide, and hydrochlorothiazide, are used to treat edema and hypertension
  • Rifampicin and rifabutin, which are used to treat tuberculosis
  • Carbenoxolone, which is used to treat mouth ulcers
  • Anticholinergic medicines used to treat myasthenia gravis, such as distigmine and neostigmine
  • Acetazolamide, which is a medicine used to treat glaucoma or edema

Your doctor may need to adjust your medication doses or switch you to a different medication to avoid interactions with Roxadex Injection.

Contraindications
  • Gastric and duodenal ulcers.
  • Systemic and ophthalmic fungal infections.
  • Viral infections, e.g., varicella and herpes genitals infections. Viral infections of the eye.
  • Glaucoma.
  • Hypersensitivity to corticosteroids
Pregnancy & Lactation

When taken during pregnancy, corticosteroids can have negative effects on fetal development, including orofacial clefts, growth restriction, and decreased birth weight. It is recommended that females of reproductive potential undergo pregnancy testing before starting treatment and use effective contraception during treatment and for at least one month after the final dose. In some patients, steroids may affect male fertility by increasing or decreasing sperm motility and number. In lactating women, systemically administered corticosteroids can be present in breast milk and may interfere with the growth or production of natural corticosteroids in the infant. As a result, it is advised that women do not breastfeed during treatment and for 2 weeks after the last dose.
